Goal:
1 - This small survey on the effects of COVID-19 vaccines on people with CML, does not concern the general public. Also, we ask anyone who does not have CML to refrain from participating. This survey is not linked to any government agency or to a patient association. If the participation is sufficient, the results will be published. No one is forced to participate.
2 - This survey will be short-lived. Its purpose is to inform people with CML about the short-term effects of vaccines and possible interactions with treatments. Patients should not rely on this survey to make a decision about whether or not to get the vaccine. Patients should discuss the dangerousness of vaccines with their hematologist.

3 - Since this survey is linked to medical data, it must be subject to maximum confidentiality criteria. To do this, no data from this survey should allow the person to be identified (no name, no civic address, no email address and no complete dates (only the year, the month and not the day). In order to meet this requirement, the "CML Permanent Code" has been specially designed to ensure data confidentiality. In addition, these two codes, the "CML Permanent Code" and the "CML Medication Status", must ensure that this data is error-free through a validation procedure. This validation will prevent forms from being rejected due to errors during data entry. By pressing this button, you can get an idea of the content of the survey results file.
To be able to complete this survey, you must first create your CML Permanent Code as well as your CML Medication Status.
After their creation, you can transfer these codes to this form by copying / pasting them.
For more information on data confidentiality, see the 'CML Permanent Code' module.
